I've been studying piano, harmony, counterpoint, orchestration, and arrangement since my childhood. I've transcribed and arranged over 200 pieces so far. Some examples are orchestral works, string quartets, jazz trios, virtuoso piano arrangements, piano cadenzas, virtuoso jazz playing, etc.

It depends. Slow songs with little movement, one-note instruments (for example, flute or violin), and songs with lots of repetition will cost $5 per minute. As a rule of thumb, the faster the song, the more notes I'll have to individually transcribe, and thus the more I'll have to charge.

Yes. The higher the number of notes, tempo (speed), and/or the number of instruments, the harder it will be to transcribe or arrange the song.
